Secure Access to Your Visit & Records

We use a secure, HIPAA-compliant patient portal powered by CharmHealth.

This portal allows you to complete required forms, access visit documentation, and securely communicate regarding your scheduled evaluation.

Through the patient portal, you can…

  • Complete intake questionnaires and consent forms

  • Upload relevant medical records (optional but recommended)

  • Attend your scheduled telehealth visit

  • View visit summaries and clinical documentation

  • Receive administrative messages related to your appointment

  • Access invoices and receipts for self-pay services

The portal supports the following evaluation services

Before Your Visit

  • Complete all required intake forms at least 24 hours before your appointment

  • Upload any relevant records if available (optional)

  • Ensure you can access video and audio from a private location

After Your Visit

  • Visit summaries and documentation will be available in the portal

  • Administrative follow-up (if needed) will occur through secure messaging

  • No further action is required unless instructed during your visit

  • Initial medical cannabis eligibility evaluations

  • Renewal evaluations

  • Consultative evaluations by physician referral

  • Extended consultative visits when clinically appropriate

  • Documentation-based outcomes (e.g., clinical summaries or letters when applicable)
