We use a secure, HIPAA-compliant patient portal powered by CharmHealth.
This portal allows you to complete required forms, access visit documentation, and securely communicate regarding your scheduled evaluation.
Through the patient portal, you can…
Complete intake questionnaires and consent forms
Upload relevant medical records (optional but recommended)
Attend your scheduled telehealth visit
View visit summaries and clinical documentation
Receive administrative messages related to your appointment
Access invoices and receipts for self-pay services
The portal supports the following evaluation services
Before Your Visit
Complete all required intake forms at least 24 hours before your appointment
Upload any relevant records if available (optional)
Ensure you can access video and audio from a private location
After Your Visit
Visit summaries and documentation will be available in the portal
Administrative follow-up (if needed) will occur through secure messaging
No further action is required unless instructed during your visit
Initial medical cannabis eligibility evaluations
Renewal evaluations
Consultative evaluations by physician referral
Extended consultative visits when clinically appropriate
Documentation-based outcomes (e.g., clinical summaries or letters when applicable)
